PAYMENT POLICY

Medical services are provided and billed by Sunflower Pediatrics. Our providers participate in almost all of Georgia's major health insurance plans. Following are the health plans which we currently accept.

If your plan is not listed, we will be unable to file a claim on your behalf. However, we can provide you with a form to submit to your insurance company to instruct them to send reimbursements directly to you. We presently do not accept HMOs, including BCBS HMO, or BCBS POS plans. Please be aware that there may be deductibles and co-payments with any health plan for which you could be responsible.  For the most up-to-date information on our accepted private health insurance plans

Additional fees

There is an additional $25 service charge for any returned checks and any bank fees. We have set aside time, especially for your child. Therefore, we will apply an invoice for failing to keep appointments without 24 hours prior notice: $25 for Sick visits, $50 for Well visits/checkups, and ADHD appointments per child.

We will provide a copy of your child's vaccine record at no cost. There is, however, 93 cents per printed page charge, per chart, plus postage for copying medical records, payable in advance. The minimum charge per chart is $15.00. It will typically take one to four weeks to fill these requests.
